Please download and fill-out the below waiver form for the 3 attractions (Euro Bungee Trampoline, Hungry Hippo Challenge and Inflatable Slide).
Once the form is signed, it can be turned into the Windmill front office or emailed to [email protected] (Carla Mosthaff).
Wristbands will be given out on 10/11/24 to indicate that the waiver form was turned in.
